4- Gender (male, female, transgender, etc.)
5- Specific age range (e.g., between 13-17 years of age; 35-50 years of age)
6- Specific community/county/ZIP code of focus (e.g., Rockledge high school, Flagler County, etc.). *Choosing a specific area helps streamline data and subsequent details that will be required in future assignments. Choose an area within ten miles of your current residence or for which you have family or plan to work.
7- An Aggregate Topic Statement that must be written in a proper format. Format examples include:

a slide with a summative introduction including details of the applicable Community Health Needs Assessment (CHNA) results that support the selected topic and related disparity(ies): specific population, related health disparity, focus on prevention or management, and potential impact on the population. (Summative introduction should be no less than size 20 font and no more than one slide.)
a slide with CHNA results and key information related to the aggregate’s health status in the community.
a slide with a properly formatted aggregate topic statement including proposed strategy (e.g., management of, reduction of, prevention of, etc.).
