Our idea is based on trying to improve documentation and initial assessment when it comes to wounds, especially when an admission comes and the nurse isn’t sure how to stage it. And because the nurse may not be quite sure on how to stage it, it could lead to false documentation of what’s on the patient’s skin. Another issue that might arise is who to consult for whatever is on the patients skin. For example, someone might consult wound care for something that should be dealt with by podiatry. So with our QI project, the aim is to educate the staff on wounds and how to properly stage wounds on patients. The idea is to get the staff’s baseline knowledge on wounds by showing them pictures of wounds and asking them how they would document/stage the wounds. And with that data, we can get a baseline on how well the staff can stage wounds. To measure progress, our idea is to possibly go around and educate nurses on wounds and then test their knowledge by going around and asking them how they would stage/document on what’s on the patient’s skin. And by the end of the education, RNs will also be able to properly consult the right team (podiatry/wound care/derm).
